Service Description

Continued follow up with health coach to help you to implement and sustain your care plan. Working alongside your Nurse Practitioner lead team, your health coach helps translate medical recommendations into practical, everyday actions. The visit will focus on nutritional guidance, lifestyle, stress, sleep and habit change, helping you with accountability, education, and realistic strategies to help you reach your health goals.
